Registration and Intake Assessment Forms
At STA, where your concerns deserve special care, we have designed our proprietary forms to begin an in-depth assessment. The forms can be emailed or faxed at your convenience to registration@specializedtherapy.com or to 201-342-4346 attention Intake Department. You may also bring your forms with you to the appointment.
Your therapist will review your forms and then ask a series of questions designed to help formulate a recommendation and initial treatment plan. Discuss your treatment plan with your therapist.

STA participates in all insurance companies but due to limitations set by some insurance companies not all providers are in network with all insurance companies. We would gladly participate as a group with all insurance companies but a few companies, such as United, don't allow all providers to participate. When you register with us, we will do an insurance verification to check the status of participation for your policy.
